After 48 wonderful years of practicing medicine in the same location, I am writing to share some important news. Due to changes in my building's leasing policy, I must vacate my office by June 30, 2025.
While this transition comes sooner than I had planned, it feels like the right time to close this chapter of my professional life. To my longtime patients, you have been like family to me, and I will miss you deeply. To those who joined my practice more recently, I'm sorry to place you in the position of finding a new primary care doctor so soon.
Next Steps for Your Care
- Choose a New Physician: Attached is a list of recommended primary care practices for your consideration.
Primary Care Doctors - Request Your Medical Records:
- If You Have Chosen a New Physician: Once you are ready to transition care, please visit DesertRiverSolutions.com to create an account and request your medical records. If you cannot access a computer, contact our office, and we'll assist you. Please make your records request at least 4 weeks before your first appointment with your new doctor.
- If You Have Not Chosen a New Physician by June: All remaining medical records will be securely transferred to Desert River Solutions between June 1-10. You can request your records at any time after this through their website, DesertRiverSolutions.com.
Desert River Solutions will manage requests for records both now and in the future. Once your records are digitized, they will be provided electronically or via thumb drive.
I recommend meeting with your new physician before May 31, 2025, to ensure a smooth transition of care.
Practice Transition Timeline
Now - May 31, 2025
In-Office Visits & Administrative Services: Services as usual.
Plan Your Transition: Schedule an initial appointment with your new physician.
If your annual exam falls during this time, you may prefer to do it with your new doctor.June 1 - June 30, 2025
Telehealth & Administrative Services Only: No in-office visits.
Our participation with Medicare will end on May 31: Telehealth services after June 1 will not be covered by Medicare.
Reschedule Appointments: Any existing June appointments should be moved to your new physician or changed from in-office to telehealth.
Starting July 1, 2025
Telehealth Consultative Services: I will be available for telehealth consultative services.
Eligibility for Telehealth Consultations:
- You must have been a patient seen by me after January 1, 2020.
- You must have an established relationship with a new primary care physician.
Additional information regarding Telehealth Consultative Services:
- Understand that I can only offer advice appropriate for telehealth.
- Understand that services will not be covered by Medicare.
- Non-Medicare patients will be provided with a receipt that may be submitted to their insurance company.
- Once digitized, I will have access to your medical records.
- Consultations will be provided, according to your preference, by telephone only, FaceTime, or Zoom.
